IMPORTANT! Documentation Needed For Dispensing Medicine to Students at School
Parents and Guardians
If your child attends Smiths Station High School and will be taking their prescribed medicine during the school day. The Lee County Student Medication form must be on file at the Smiths Station High School. The form must be completed and signed by both the student and their physician.
If the student does NOT have this completed and signed form submitted and on file, the school nurse will not be allowed to dispense your child's medicine to them.
Please submit this completed and signed form along with the medicine that is prescribed for your child to the SSHS Nurse as soon as possible.
